Whether you are joining us or moving to another practice, here is how to move your records.

Joining St Ives Doctors

Bringing your records to us.

We can request your records from your previous practice for you. Complete our online form and we send the request on your behalf. We do not charge to receive your records.

  1. Complete the form

    Tell us who you are and which practice holds your records. A parent or guardian completes it for a child.

  2. We send the request

    We send your signed request to your previous practice. They may contact you about a fee or a form of their own.

  3. Your records come to us

    We ask for an electronic file in XML format, sent to records@stivesdoctors.com.au, so your history loads straight into your record here.

You do not need to wait for your records before seeing us. If you have a new or pressing concern, book an appointment and bring a list of your medicines.

Leaving St Ives Doctors

Sending your records to another practice.

The quickest way to transfer your records is to complete our online form. It collects your authority, your details and the payment in one step, and we then prepare your records and send them to your new practice.

The form opens in a new window. Payment is taken securely by card as part of the form.

  1. Complete the form

    Tell us who you are and where your records are going. A parent or guardian completes it for a child.

  2. Pay the $30 administration fee

    The fee is paid by card when you submit the form. It covers preparing and sending your records, and has no Medicare rebate.

  3. We send your records

    Once we have your form and payment, we send your records securely to the practice you have named, within 10 days.

Why is there a fee to send my records?

The $30 administration fee covers the staff time to prepare, check and send your records. It applies only when records leave the practice.

Can I have a copy of my records for myself?

Yes. You are entitled to see your record or have a copy. See seeing or correcting your information in our privacy policy.

Can I keep seeing my doctor here after a transfer?

Yes. Sending a copy of your records elsewhere does not close your file with us.
